Well, it is quite simple if you have some basic knowledge of max... make the structure that will be your wall (usually you will start with a box and then enlarge at your like), increase the number of divisions of the box, then convert it to a mesh (right click on object and choose convert to mesh), once done, you can edit the mesh removing the face, vertices or poligon where your door will be. That's it, there you have a hole into the wall. Just create the door and position it in the hole.
